Preparation Guide & Forms
Dear Birch Rock Family:

We are truly thrilled about the upcoming summer. We’ve got a sensational group of campers and staff heading up to the Rock. At this point we’re all counting the days to Opening Day.

The Birch Rock Camp Family Guide packet contains all forms and information needed for the summer.

Below are all the included documents and a link to download each individually:

Baggage and Shipping Form We only require this form if you are shipping to or from Birch Rock. If your son will be arriving with all of his things on Opening Day, this form will not need to be filled out.
Community Agreement The Community Agreement is a form that needs to be signed by both the parent and the camper stating that you have thoroughly reviewed and understand our community policies found in the Family Guide.  This form also gives permission for participation in activities, target sports, trips and, most importantly promotional permission to use a photo of your son and your son's first name and first initial of his last name in camp advertising materials, or on our website, etc. We must have this form in order to display any pictures of your son on our website’s camp journal.
Emergency Contact Sheet This form is used by the camp’s summer office. It is the best way for Birch Rock to ensure that our contact information for your family is up to date, in case something has changed since the registration.
Family Guide This guide will answer many questions for both parents and campers about the upcoming summer. It also includes many of our policies.
Flight and Transportation Form and Photo We only require this form if you will not be dropping your son off directly at camp. If your son is flying or taking the train and will need the camp to pick him up in Portland, we will need this form with a photo of your camper, and would also love to have a copy of the flight itinerary if possible.
Maine Camp Outfitters Catalog Birch Rock shirts will be required each day for dinner and on all off-campus trips. Khaki shorts/pants will only be required on Sundays and other special occasions. The camp’s hooded sweatshirts are very popular with the kids in the morning and on chilly days. Please be sure to review the catalog.
Medical Form This form is a three page Health History with a fourth page for the Health Exam. The Health History section is filled out by the camper’s parent and needs to be updated every summer. The fourth page, the Health Exam, is filled out by your son’s doctor and needs to be updated at least every other summer. A photocopy of the front and back of your medical insurance card must also be mailed in with the Medical Form.
Parent Questionnaire Form This is the form we review closely with our staff during Staff Orientation week. The answers you provide to these questions will help us better understand your son(s) and what you hope for him to get out his summer experience.
Recommended Packing List This list will help you pack and prepare for the summer. We highly recommend that you pack extra socks.

It is important that you review and discuss these materials with your camper(s) prior to his arrival at Birch Rock, and that you sign and return the necessary forms to camp.

It is important that all these forms be mailed to us by June 1st. Without these forms our staff will not be able to appropriately prepare for your son’s exciting upcoming summer. Campers will not be eligible to participate until all forms and payments are received.
